Our Intake Process

1. Initial Contact & Scheduling

Receive inquiry or referral (via phone, email, or event). Schedule an intake meeting at a convenient time for the client.

2. First Meeting & Rapport Building

Welcome the client and explain my role as case manager. Explain the mission of Authentically Aligned Living LLC. Clarify the purpose and structure of the intake process.

3. Presentation of HIPAA Privacy Practices

Provide the client with a copy of HIPAA privacy practices. Verbally review the main points, including confidentiality and client rights. Answer any questions about privacy and data protection. Obtain the client’s written acknowledgment of receipt.

4. Consent to Release Information (ROI)

Explain the need for a Release of Information (ROI) form to coordinate care with other providers (doctors, therapists, insurance, etc.). Review the specifics: what information will be shared, with whom, and for what purpose. Ensure the client understands and answer any questions or concerns.

5. Comprehensive Needs Assessment

Conduct a structured needs assessment covering: Medical history and current health concerns Social determinants (housing, employment, support systems) Insurance and financial status Goals, preferences, and immediate needs Use a standardized assessment form for consistency.

6. Documentation & Next Steps

Document all collected information in the client’s record. Summarize key findings and proposed next steps to the client. Schedule a follow-up appointment or next action. Provide the client with contact, appointment information, and a summary of what to expect.
